How to Order From Us
We have several forms. You don’t need to complete every form—review what applies to you and choose accordingly.
For First-time customers (or changes to your pharmacy)
If this is your first order or if there’s been a change of manager, owner, or pharmacy name, please also complete these one-time items:
One-time setup requirements
- Complete the Sign-Up Agreement. Required under Health Canada Policy 51 and College of Pharmacists of Manitoba regulations.
- Complete the Payment Form (card on file required).
- Fax both forms to 1-833-536-1732.

Step 1 — Decide what you need
A) Just a price quote (no commitment)
- Complete the Price Quote Request form.
- Fax it to 1-833-536-1732.
- We’ll reply with a price (typically within 24 hours).
- If you like the price, check the box and fax back again.
B) Place an order now
- Complete the Order Form.
- By submitting, you agree to proceed at our default price.
- Fax it to 1-833-536-1732.
C) Diclofenac bulk orders
- Complete the Diclofenac Bulk Order Form.
- Fax it to 1-833-536-1732.
