Hormones​

DHT causes gender pattern baldness by “miniaturization”; each cycle makes the hair thinner, shorter, and lighter, and it may turn “peach-fuzz” or fall out completely over time. Androgen or androgenic is another word for DHT affect.

Thyroid changes, postpartum/menopause, or starting/stopping birth control can trigger temporary shedding and may warrant lab checks.

Growth Phase Changes​

More hairs shift from the growth phase (anagen) into the resting/shedding phase (telogen). If this is following stress, illness, childbirth, or rapid dieting and is often temporary.

Inflammatory Damage​

What is it

What is it

Alopecia areata is an autoimmune condition where the immune system mistakenly attacks the hair follicles, leading to patchy hair loss. Some forms cause complete body hair loss called alopecia areata universalis. 

How do we treat it

Topical corticosteroids help by:

Suppressing Local Immune Attack

Corticosteroids reduce the activity of T-lymphocytes around the hair follicle.

This prevents immune cells from targeting and damaging the follicle.

Reducing Inflammation

They decrease the release of pro-inflammatory cytokines (messenger molecules like IL-2, TNF-α).

Less inflammation means follicles can return to a healthier environment.

Promoting Follicle Recovery

By calming inflammation and immune activity, the follicle can re-enter the anagen (growth) phase and begin producing hair again.​

What is it

This is a reactive shedding condition where many hairs prematurely shift into the telogen (resting) phase after a trigger (stress, illness, postpartum, nutritional deficiency, medication, etc.). If usually presents within 6 months of the stressful event.

How do we treat it

Since this is caused by a event in the past, treatment is focused on resetting your hair growth and providing nutrients to the scalp.

Minoxidil
What it is

Minoxidil was first marketed and used for high blood pressure. It was then noted that people taking it were experiencing hair growth. Since then it is the staple and foundation of hair loss treatment. 

Treatment protocol are built around it and it is used in every type of hair loss. 

How it works and who it's for

It is what we call a vasodilator. It works by increasing blood flow by dialating (opening) the blood vessels so more blood, and therefore nutrients are delivered.  It also stimulates hair follicles & prolongs growth phase. In telogen effluvium, it specifically works to “reset and coordinate” the hair growth cycles so all the hair grows together. 

Oral Vs. Topical

Topical minoxidil

Pros:

  • Avoids systemic absorption = less/no changes in blood pressure
  • Available over the counter with no prescription
  • Can apply to specific areas for different results (eye brows, beard)
  • Increases in effectiveness when combined with microneedling

Cons:

  • Applied twice daily – can be hard to commit to
  • Toxic to pets – must be very careful that pets do not come in contact with it and your hair (until an hour after you apply it)
  • Can cause hair growth on normal skin if exposed (wipe off of your forehead to avoid unwanted hair)
  • Can cause scalp sensitivity/ allergy. Often contains alcohol which can dry out the scalp
  • Can be expensive
  • Exact dosing is harder to achieve with topical application
  • May affect ability to style hair

Oral Minoxidil

Pros:

  • Once a day dosing makes using it regularly easier
  • Can be more effective than topical
  • Can be more cost effective
  • Doses are very consistent
  • Helps hair growth in other areas (eye brows, beard)
  • Safe around pets and children (if stored away from reach)
  • No residue left over and can style hair freely

Cons:

  • Requires a prescription
  • Not suitable for everyone as can decrease blood pressure
  • Can cause unwanted hair growth (irreversible and requires laser hair removal) 
Adverse Effects

Topical : Burning, itching, redness

Oral: Low blood pressure, dizziness, tachycardia (fast heart beat), hypertrichosis (excessive hair growth, may be in unwanted areas), edema (swelling caused by water retention)

Note: This is not an exhaustive list of any and all potential adverse effects. 

What it is

Both of these medications are usually used for BPH (benign prostatic hyperplasia). Propecia is the name brand of finasteride 1mg pill.  Dutasteride is in the same drug class as finasteride but is a stronger version that is sometimes used if finasteride isn’t working. Dutasteride is not usually used topically as it is a much larger molecule and has more difficulty absorbing. 

How it works and who it's for

They work by attaching, and stopping the enzyme 5-alpha reductase from converting testosterone to dihydrotesterone (DHT). DHT is 2.5-10 times more potent than testosterone. DHT binds to the hair follicles and causes miniaturization of the scalp hair. 

Oral Vs. Topical

Topical Finasteride

Pros:

  • Avoids systemic absorption = less/no changes in testosterone dependent affects. Lower risk of sexual disfunction. 
  • May be used in females if they are post-menopausal
  • Reduces the risk of negatively affecting beard growth
  • Increases in effectiveness when combined with microneedling

Cons:

  • Can cause scalp sensitivity/ allergy. Often contains alcohol which can dry out the scalp
  • Needs to compounded (has to be made at specialty pharmacies)
  • Can be more expensive
  • Exact dosing is harder to achieve with topical application
  • May affect ability to style hair

Oral Finasteride

Pros:

  • Pill for can makes using it regularly easier
  • Can be more effective than topical
  • Can be more cost effective
  • Doses are very consistent
  • No residue left over and can style hair freely

Cons:

  • Increased risk of adverse effects like sexual / erective disfunction 
  • Not suitable for women
  • Can cause decrease in beard growth
Adverse Effects

Topical : Burning, itching, redness, sexual disfunction (lower risk)

Oral: Sexual disfunction (reduced libido, erective disfunction, ejaculation disfunction), low blood pressure

What it is

Steroids’, specifically corticosteroids are immunosuppressant medications that work to reduce inflammation.  

How it works and who it's for

This is the main treatment for alopecia areata. This can be scaring or non-scaring. This condition is caused by the body mistakenly “attacking” the hair follicles and causing hair loss by inflammatory processes.

Inflammation works by using signals to flag the immune system to attack.

1) It increases blood flow to allow white blood cells and inflammation “signals” to enter the location and attack.

2) Causes the cells to release even more “signals” that to the immune system to come and attack.

Steroids enter cells and change how genes (DNA) are expressed (the cells orders). This stops the orders to make the pro-inflammation signals (cytokines, prostaglandins, and leukotrienes) and prevents more white blood cells from coming to attack. All of this reduces pain, swelling and the damage to the hair and scalp.

Topical vs. Injection

Topical and injectable steroids are used in tandem to reduce inflammation.

Injections:

  • In office procedure to reduce inflammation fast
  • Can help but the brakes on the hair loss

Topical:

  • This absorbs into the scalp on the area it is placed, usually in the morning
  • Can be used on its own for less severe inflammation, or as maintenance (keep the brakes on) after injections. 
Adverse Effects

Injection: Injection site pain, swelling, bruising, temporary increased blood sugar. Less common are injections at the site, high blood pressure, fluid retention.

Topical: Redness, irritation, burning, water retention. Do not get in eyes.

What it is

Is a anti-dandruff shampoo that has recently gained popularity in hair growth treatment. Not only does it treat active seborrheic dermatitis (the inflammatory type of dandruff, it has also been shown to help reduce hair loss and promote hair growth in recent studies.

How it works and who it's for

Ketoconazole plays two roles in hair regrowth

  1. It reduces inflammation. This is via its anti-fungal properties. Even without visible dandruff, Malassezia, the natural yeast on our scalp can cause inflammation that damages the hair follicles. 
  2. It has anti-androgenic properties. It has been shown to reduce levels of DHT in the scalp.

It is usually applied to the scalp, then let sit for at least 5 minutes. You may use conditioner after. Instructions may vary based on your provider.

It is used for AGA and inflammatory alopecia. 

Adverse Effects

Ketoconazole shampoo is very well tolerated. You may experience burning, itching, irritation and oily hair. 

What it is

Spironolactone is usually used for heart failure and lowering blood pressure. It is used off label in women for acne and hair loss. Note: This medication is typically contraindicated in men (unless used for heart failure).

How it works and who it's for

For hair loss (and acne) we start at low doses of spironolactone for its anti-androgen effect. Dosing may be increased for effect if it is being tolerated.

There are a few different mechanisms of how it reduces androgens

  1. Slows the production of testosterone
  2. It promotes testosterone to convert to estrogen
  3. It blocks testosterone from turning into DHT 
  4. It blocks androgens from attaching to the receptors in the hair follicle that cause hair loss
Adverse Effects

It may cause dizziness, drowsiness and increased urination. It can also cause hyperkalemia (high potassium), gynecomastia (benign breast growth in males), and very rarely harm the kidneys.  

What it is

Hormone replacement therapy (HRT) is a treatment that supplementing hormones that are at lower than wanted levels. For women, female hormones decrease during menopause. This can lead to many symptoms such as hot flashes, vaginal dryness and hair loss. Treatment is extremely individualized to the specific needs and goals of the patient. 

How it works and who it's for

Hormones are typically replaced by taking pills or with topical treatments.

Estrogen and progesterone (the 2 main female hormones) help balance out androgens (male hormones). As female hormones decline the male hormones have a stronger effect on the body. Male hormones lead to miniaturization of the hair follicles and hair thinning and loss. 

Adverse Effects

Adverse effects are treatment specific and should be discussed with your provider for your specific treatment. 

In general estrogen causes the uterine lining to grow, which increases the risk of endometrial cancer. This risk is reduce/eliminated by taking progesterone. 

There has been a lot of media coverage about the harms associated with HRT, especially about breath cancer and heart health. These claims are now being proven incorrect and HRT is shown to reduce the risk of breast cancer and improve heart health. 

Discussing with your provider the benefits and risks of HRT is the best way to know if it’s right for you.

What it is

A healthy diet and having the right vitamins and minerals is essential for health hair. We know that lacking certain things like iron, vitamin D, and vitamin C are major factors in hair loss. There is substantial evidence that taking vitamin D, vitamin C and iron help with hair growth. 

How it works and who it's for

Nutrients are the “fertilizer” of your hair. If we increase the blood flow (minoxidil) but don’t give the nutrients, hair won’t grow back as health and strong. 

Priorin (Hair, skin and nail vitamin)

  • Studies have shown Priorin increases hair density and improves subjective perceptions of hair quality and appearance, including shine and strength. High quality evidence is rare to find. This is the best supplement we have found so far. 
  • Millet Extract: A natural source of minerals and essential fatty acids, it nourishes hair follicles and protects the scalp.
  • L-Cystine: An amino acid crucial for protein formation in hair, it helps create stronger, more resilient hair strands.
  • Calcium Pantothenate (Vitamin B5): Supports a healthy scalp and the overall hair growth process.
  • Wheat Germ Oil: Provides Vitamin E and essential fatty acids, offering antioxidant benefits and improving scalp circulation to deliver nutrients to hair follicles.
  • Note: We have no, nor have ever had a relationship or association with the makers of Priorin. We receive no incentives. This is the best supplement we have found so far. Anecdotal evidence from our own patients have shown positive results. 

Vitamin D3

  • Supplementation is already recommended for everyone in Canada for good reason, most people are deficient due to our long dark winters. 
  • Vitamin D directly interacts with vitamin D receptors on hair follicles promoting hair growth.
  • When levels are low, our hair responds by shortening the growing (anagen) phase, supplementing it lengthens the anagen phase.

Vitamin C

  • Vitamin C deficiency isn’t as common as it was in the sailing days, but with increases in processed foods and higher grocery prices we are finding people don’t have enough for great hair. 
  • Vitamin C is a major component of collagen. Collagen turns into keratin (aka hair). More collagen = stronger hair which is less prone to breakage.
  • The antioxidant properties of vitamin C protect the hair from free radical oxygen species (unhappy molecules which attack everything around them). It protects the hair follicles from pollution, DNA damage and the normal affects of aging.
  • We recommend sublingual drops as some studies suggests better absorption.
  • Increases iron absorption (below)

Iron

  • Iron is a major nutrient needed to create the red blood cells that carry oxygen to our body, and our hair. 
  • Low iron and anemia should be evaluated and treated as part of a comprehensive hair loss assessment, and supplemented as needed.
  • Iron should not be taken without bloodwork. Iron can be toxic if not taken correctly. 
Adverse Effects

Vitamin C: Do not take more than 1-2g daily. Toxicity looks like gastrointestinal symptoms (nausea, vomiting, diarrhea), headache, trouble sleeping, and fever.

Iron: Iron commonly causes stomach upset, constipation, and dark stool (poop). It is located behind the counter at pharmacies. This means you have to speak to the pharmacist before buying it. Only take if directed to.

Tretinoin

Enhances minoxidil absorption and converts some minoxidil non-responders into responders. Can also be used in alopecia areata.

What it is

Microneedling is the process of making small, shallow wounds in the skin to promote the bodies natural healing mechanisms. Tiny needles are used to poke holes into the scalp. 

How it works and who it's for

Microneedling is indicated for all types of hair loss. 

Benefits

  • Increases absorption of topical treatments. Microneedling + topical minoxidil has been shown to be more effective than either alone.
  • Increases the effectiveness of steroids for alopecia areata
  • Stimulates growth factors as part of the natural healing process. This promotes blood flow and nutrients to the area.
  • Activates cell processes. Fibroblasts, elastin, and collagen are produced, these improve the quality of the skin and are components that create hair. The body’s own stem cells are also activated, further stimulating hair growth. 
Stem cell Microneedling

The same benefits of micro needling + the advanced benefit of stem cell therapy

  • The addition of stem cells promotes the reactivation of dormant hair follicles, allowing bald spots to regrow hair.

PRP Microneedling

Platelet Rich Plasma (PRP) is an in office procedure where blood is drawn, spun down and then reintroduced into the same person. This gives a nutrient boost for hair growth.

  1. A small amount of blood is drawn and centrifuged so that it separates into the different layer and components. One of these layers is PRP.
  2. It is then reintroduced by microneedling or injection into the scalp
  3. This actives dormant hair follicles and creates the right environment for denser and thicker hair. 
  4. Reverses miniaturization
  5. Low risk of reaction as it is the person’s own blood being reintroduced
Plain Microneedling

Micro needling on its own is a more cost effective way to augment hair growth. After micro needling it is recommended to wait 24 hours before restarting topical treatment. The specific instructions will be provided to you by your clinic.

Adverse Effects

Micro needling done in office has a very low risk of adverse effects. 

  • Discomfort, pain, swelling, and a small amount of blood is expected from the treatment. Your clinic may provide numbing to help with the discomfort.
  • Injection, risk is greatly reduced by following procedures provided by clinic, and avoiding at home treatments.
  • Scarring or hyperpigmentation may result from the improper use of micro needling and needles that are too long.
  • Increased sun sensitivity.
  • Flare up of cold sores.
What it is

Low Level Light Therapy is an FDA approved treatment for hair loss. Light is emitted by either lasers or LEDs that penetrates the skin, and activates growth cells. It is typically a form of wearable cap that is used daily or every other days for around 20 minutes. Devices vary in price and usage.
